Existing AMP claim Follow-Up Form

This form will enable you to send a request to the AMP Claims team to enquire on an existing AMP Home & Contents claim or an AMP Personal Motor Vehicle Claim.

Following receipt of your enquiry, an AMP Claims consultant will contact you within 48 business hours between 8am and 6pm EST. Please indicate if you are happy to be contacted outside these hours. A copy of the information that you have submitted will also be sent to your nominated email address and to your AMP Adviser. When the AMP Claims consultant does contact you, they will need to confirm your identify before they will be able to discuss the claim with you.

When completing this Follow-up Form, please supply at least two contact numbers
